Supporting Those with Serious Mental Illness through Enhanced Primary Care

Individuals with serious mental illness (SMI) experience numerous barriers to quality health care, such as poorly controlled chronic diseases, low rates of preventive care services and vulnerability to violence, stigma and marginalization. As a result, this population faces high prevalence of chronic diseases and poor life expectancy and other health outcomes. Because primary care providers are often the first to identify and support individuals with SMI and chronic diseases, integrating primary care and behavioral health care is necessary to narrow health disparities for patients with SMI. Although primary care providers are in a unique position to effectively treat physical and behavioral health conditions among this population, successfully delivering care is not well understood.
