Provides behavioral health and primary care with continuous care and support for those who have been evaluated with depression or anxiety. Mental health screenings are available, if diagnosis involves psychiatric care a referral will be made to other community services. The program will provide care management for treatment such as doctor appointments, prescriptions, and follow-ups.
