Provides treatment, rehabilitation, and support services for adults who live with severe mental illness, use drugs or alcohol, and experience one of the following: multiple hospitalizations/psychiatric emergencies, frequent involvement with the criminal justice system, or long-term homelessness.Offers the following services from a multi-disciplinary team: case and medication management, in-home rehabilitative nursing services, housing search support, assistance with legal matters, counseling, and 24-hour mental health crisis support.Serves clients in their home or in the community.
