Provides enrollment activities for individuals entering the programs, conducts person-directed planning meetings, coordinates the individual life plan of care with the provider representative, and provides on-going monitoring of provisions of the individual’s program services. Individuals will be assisted in ensuring that they are placed on the state-wide waiting list; when authorized for a slot, the MRA service coordinator will assist the individual in locating information about providers and will conduct necessary enrollment activities in coordination with the chosen provider organization.
