Provides wraparound coordination services tailored to each patient’s needs. Services may consist of ongoing engagement, case management, medication management, psychosocial rehabilitation, transportation, and connections to other community resources. Aims to reduce emergency detentions and the subsequent use of emergency and inpatient services. The team will meet callers where they are at including hospitals, shelters, boarding homes, jail, and community settings.
